The Medicaid Congress is excited to be able to offer a special preconference on Managing "Super Utilizers" - Six Sites Adapt the Camden Coalition Model, chaired by Susan Mende, BSN, MPH, Senior Program Officer at the Robert Wood Johnson Foundation (RWJF). The Foundation is helping six Aligning Forces for Quality (AF4Q) sites adapt the innovative model developed by Dr. Jeff Brenner and the Camden Coalition. The preconference will provide an update on the work of the Camden Coalition, an overview of the RWJF initiative, and progress reports form the participating sites.
